Mobile NDIS Behaviour Support in Fremantle

Positive Behaviour Support is most useful when it reflects what is happening in real life. Our practitioners work in homes, schools, community settings and workplaces so assessment and strategies are grounded in the participant’s routines, communication, relationships and environment.

Behaviour Bridge supports participants across Fremantle and nearby communities, including South Fremantle, East Fremantle, North Fremantle, Beaconsfield, White Gum Valley and Hamilton Hill. Face-to-face availability is confirmed case by case according to the participant’s location, needs, NDIS funding, current capacity and practitioner fit.

Positive Behaviour Support for participants, families and teams

Support begins with understanding the purpose and context of behaviour—not simply trying to stop it. Depending on the participant’s needs and plan, our work may include:

  • Functional Behaviour Assessments
  • Interim and comprehensive Behaviour Support Plans
  • Practical implementation support and coaching
  • Family, carer and support-worker training
  • Review, monitoring and collaboration with treating teams
  • Restrictive-practice reduction and support where relevant

What support coordinators need to know

Support coordinators, participants and families can submit a referral directly. Include the participant’s Fremantle location, current support needs, NDIS funding and the people involved in their support. This helps our team review geographic coverage, practitioner fit and current capacity before confirming next steps.

Our focus is not only the written plan. We work collaboratively with the people involved each day so strategies can be understood, implemented and reviewed across the participant’s everyday settings.
