As a Support Coordinator, you’ve seen what happens when a PBS provider writes a plan and disappears. The team is left confused. The behaviours don’t improve. And you’re the one fielding the calls at 7pm.

Immediate Capacity: We are currently accepting referrals across all of Perth Metro. Priority capacity is available in the Northern Suburbs — Joondalup, Wanneroo, Stirling, Balcatta, Morley, Noranda, Inglewood, Mount Lawley, Leederville, Northbridge, Subiaco, Osborne Park, Bayswater, and surrounding areas. We also have practitioners in Sydney taking on participants on a case-by-case basis.

What Happens After You Refer

Fast, Clear, and Always Followed Through

We don’t leave you guessing. Once you submit a referral, here is exactly what happens:

  1. Review (Same Business Day): We review the referral on the same business day to ensure we have the right clinical match and capacity for the participant’s specific risk profile. You will hear from us the same day.
  2. Initial Contact: We reach out to the primary contact (family or support coordinator) to discuss the current situation and immediate risks.
  3. Service Agreement: We provide a clear, transparent Service Agreement outlining the hours required for the Functional Behaviour Assessment and initial implementation coaching.
  4. Assessment Begins: The initial assessment is booked within the first 2 weeks of the service agreement being signed. We schedule the first in-home or community observation to begin the B.R.I.D.G.E process.
