How It Works

A Simple Path to Quality Support Of Life

1. Submit a referral

Send through a referral or enquiry and we will contact you within two to three business days to chat about your needs and confirm if Behaviour Bridge is the right fit.

2. Initial consult

We meet with the person and their key supports to understand what daily life looks like, what is currently hard, and what a better quality of life would feel like for everyone involved.

3. Assessment

This may include observations, collecting background information, reviewing incident data, and speaking with the support team to understand triggers, patterns, and the purpose behind behaviours.

4. Behaviour Support Plan & coaching

We develop a practical and easy to follow Behaviour Support Plan and coach the people around the person to use strategies with confidence and consistency.

5. Ongoing implementation & reviews

We stay alongside you as strategies are implemented, adjust what is not working, and review progress regularly to make sure quality of life continues to improve over time.

Note: Behaviour Bridge is a mobile clinic. Sessions take place in the home, community, school, workplace, or other environments that suit the person’s needs.

How long does a Behaviour Support Plan last?

Plans are reviewed regularly, typically every 12 months, or sooner if needs change.

We involve the individual, family, support workers, and relevant professionals to ensure strategies fit real life.
